A private health diary your patient can use on any phone. The patient controls the record and gives you a report when they choose.

What it is

Health Diary is a free browser-based diary for home blood pressure and blood glucose readings, built by Dr Reza Moazzeni, consultant cardiologist (Heartcare Sydney), for use in his own practice. Patients record readings on their phone; the diary produces a structured PDF — overall and recent averages, extremes with dates, morning versus evening averages, counts per standard reference range, a trend chart, and the complete log — which the patient prints or emails to you before or at a consult.
