About this sheet
Each row is one day, with space for a morning and an evening reading β the pattern most doctors ask for when reviewing home blood pressure. Write the top (systolic) and bottom (diastolic) numbers exactly as your monitor shows them, and note anything unusual in the last column. Two printed sheets cover a month.
Paper or the online diary β which is better?
Whichever you'll actually use. The advantage of the online diary is that it calculates your averages, draws your trend, and produces a report automatically β and it can't be lost or left at home. Many people start on paper and switch later.
